Position Summary

UNM’s Office for Community Health (OCH) is a key resource to New Mexico’s communities and operates under the Executive Vice President of Health Sciences. OCH’s many programs offer a diverse set of resources linking community health needs with University resources in all mission areas-clinical services, education, research and health policy. This position, also referred to as the Family Specialist, reports to the Project Director and provides health and wellness support as well as education to families in the primary care setting.

The Más DULCE program is a universal, family support, clinic and community-based intervention for postpartum people and their infants, aged 0 to 12 months. The Family Specialist is the anchor for the program in the clinic, and a team of Community Health Workers lead a peer support group component with patients in the community. As part of the daily responsibilities, the Más DULCE Family Specialist meets with families at routine well-child visits. They provide periodic check-ins between medical provider encounters via phone, text or email. During encounters, the Más DULCE Family Specialist utilizes evidence-based education and nationally approved guidelines to ensure that families are supported in caring for their children and adhering to the recommended plan of care.

The Más DULCE Family Specialist also connects families to services supporting unmet economic, social, and legal needs. The Family Specialist is also responsible for recruiting for the peer support group component, serving as a link between the clinic and the peer support team, and working collaboratively with the team. Vital to the role of the Más DULCE Family Specialist is responsibility over data collection, performing patient documentation in the electronic medical record (EMR) and in REDCap, and participating in quality improvement activities, training and national meetings as needed. This is a part-time (0.75 FTE or 30 hours per week) position with flexible hours in a clinic setting. Ideal candidate must be able to work with minimal supervision.

Conditions of Employment
Minimum Qualifications

High school diploma or GED; at least 7 years of experience directly related to the duties and responsibilities specified.

Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.

Preferred Qualifications
  • Bilingual (English and Spanish)
  • Certified Community Health Worker or Certified Peer Support Worker
  • Experience documenting in electronic medical records, REDCap or other patient data platform
