Position Summary

The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research, and Service. These centers, authorized by the Developmental Disabilities Assistance and Bill of Rights Act, build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families.

The CDD’s Medically Fragile Case Management Program (MFCMP) hires RN Case Managers to provide comprehensive case management services across New Mexico for individuals who are medically fragile and their families. The MFCMP contracts with the Medically Fragile Waiver and the MCOs. The RN Case Managers assist the families to access community based health care services, home visits, health care assessments, and assist in transition planning to community from the hospital as needed. The RN Case Manager works with the individual, family and the interdisciplinary health care team to develop a comprehensive health care plan to assist in the evaluation, implementation and coordination of services through the health care team and community based providers.

RN/case management services are provided statewide. The RN Case Manager must travel to rural and frontier areas as needed. The preferred candidate will need a minimum of two or more years in pediatric and/or community health nursing. Experience working with individuals who are medically fragile and/or with developmental disabilities is also preferred. The RN Case Manager must also have the ability to work autonomously, be a team leader as well as team member. This position travels to different regions of New Mexico.

Conditions of Employment
  • Possession of a valid New Mexico driver's license is a requirement for this job.
  • CPR Certified.
  • May be required to submit to annual TB and rubella screening.
  • UNM will provide Hepatitis vaccination, candidate may decline vaccination through declination form or may provide record of vaccination from previous employer.
  • Successful candidate must submit to post-offer, pre-employment physical examination and medical history check.
Minimum Qualifications

Associate's degree in Nursing; at least 1 year of experience directly related to the duties and responsibilities specified.Certification/LicensureState of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the multistate privilege to practice compact with New Mexico.

Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.

Preferred Qualifications

Preferred experience with:

  • Pediatric and/or community health nursing
  • Home visiting
  • Working with children/families and/or individuals who are medically fragile and/or have special health care needs
  • Working with an interdisciplinary team (IDT) as a team leader and team member
  • Working with community providers (physicians, PCPs, schools, hospitals, FIT programs, therapists, nursing agencies, State agencies
  • Accessing resources and networking with the community and the IDT
  • Working with other nurses and/or case managers in terms of training, daily work and prioritization of duties

Bilingual English/Spanish

Additional RequirementsThis position requires statewide travel to rural and frontier areas. Must pass a pre-employment criminal background check including DOH fingerprinting.
