|Position Summary||The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service. These centers, authorized by the Developmental Disabilities Assistance and Bill of Rights Act, build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families.|
The Medically Fragile Case Management Program (MFCMP) hires RN/case managers to provide comprehensive nursing case management services for individuals who are medically fragile and their families. The RN/CMs assist the individual and family to access community-based services. This is done through home visits, health care assessments, and transition planning from the hospital to their community. The RN/CM works with the individual, family, and the interdisciplinary health care team to develop a comprehensive health care plan to assist in the evaluation, implementation, and coordination of services through the health care team and community-based providers. RN/CM services are provided statewide, the case manager must travel to rural and frontier areas as needed, have two or more years in pediatric and/or community health nursing. Preferred to also have experience working with individuals who are medically fragile and their families and/or individuals with developmental disabilities as well as making home visits. The RN/CM must also have the ability to work autonomously. This position may be required to travel to different regions of New Mexico.
|Conditions of Employment|
Bachelor's degree in Nursing or a related clinical field; at least 1 year of experience directly related to the duties and responsibilities specified.Certification/LicensureState of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the multistate privilege to practice compact with New Mexico.
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
