The Center for Development and Disability (CDD), established in 1990, is New Mexico's University Center for Excellence in Developmental Disabilities Education, Research and Service. These centers, authorized by the Developmental Disabilities Assistance and Bill of Rights Act, build the capacities of states and communities to respond to the needs of individuals with developmental disabilities and their families.
CDD's Medically Fragile Case Management Program (MFCMP) hires RN/case managers to provide comprehensive case management services across New Mexico for individuals who are medically fragile and their families. The MFCMP contracts with the Medically Fragile Waiver and the MCOs. It is a requirement of the Medically Fragile Waiver and the MCOs that an RN provide this service. The case managers assist the families to access community based health care services, home visits, health care assessments, and assist in transition planning to community from the hospital as needed. The case manager works with the individual, family and the interdisciplinary health care team to develop a comprehensive health care plan to assist in the evaluation, implementation and coordination of services through the health care team and community based providers.
RN/case management services are provided statewide, the case manager must travel to rural and frontier areas as needed, preferably has two or more years’ experience in pediatric and/or community health nursing. Experience working with individuals who are medically fragile as well as having developmental disabilities is also preferred. The case manager must have the ability to work autonomously, be a team leader as well as team member. This position travels to different regions of New Mexico and will primarily cover the San Juan and McKinley counties.

Bachelor's degree in Nursing or a related clinical field; at least 1 year of experience directly related to the duties and responsibilities specified.Certification/LicensureState of New Mexico Licensed Registered Nurse or licensure pending as documented by temporary licensure OR current RN licensure from a state participating in the multistate privilege to practice compact with New Mexico.
Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience on a year for year basis.
